· 简要规则描述：对核医学科PET-CT机房注射室、缓冲、储源、更衣、护士站等区域装饰及隔墙进行拆除，新砌紧急冲淋隔墙、注射室更衣、紧急冲淋、储源缓冲区域五面防护及装饰面施工、新做示教室隔墙、墙顶面装饰、污染区域暖通风管及设备更换，强弱电、给排水配套施工，配合新衰变池施工，以便于开展相关业务并符合管理要求。
· Brief Specification Description:Dismantle the decoration and partition walls of the injection room, buffer, storage source, changing room, nurse station and other areas in the PET-CT computer room of the nuclear medicine discipline. Construct new emergency shower partition walls, change rooms in the injection room, emergency shower, and storage source buffer areas for five sides of protection and decoration. Construct new classroom partition walls, wall top decorations, replace warm and ventilation pipes and equipment in contaminated areas, and carry out supporting construction for strong and weak electricity and water supply and drainage. Cooperate with the construction of the new decay pool to facilitate related business and meet management requirements.

(a)满足《中华人民共和国政府采购法》第二十二条规定；

· (b)落实政府采购政策需满足的资格要求：推行节能产品政府采购、环境标志产品政府采购。促进中小企业、监狱企业、残疾人福利性单位发展。规范进口产品采购政策。

3.2本项目仅面向中、小、微型供应商采购。
3.3未被列入“信用中国”网站失信被执行人名单、重大税收违法案件当事人名单和中国政府采购网(www.ccgp.gov.cn)政府采购严重违法失信行为记录名单。
3.4法人的分支机构以自己的名义参与本项目采购活动时，应提供依法登记的相关证明文件和由法人出具的对本项目采购活动承担全部直接责任的授权书：自然人应提供身份证明文件。
3.5其他资格条件：（1）供应商须具备建筑工程施工总承包三级及以上资质，并具备有效期内的安全生产许可证；
（2）拟派项目负责人资格：需具备建筑工程专业二级及以上注册建造师执业资格，并具备有效的安全生产考核合格证书。
