投标人需提供以下文件
The following documents must be provided
*（1）投标人须按招标文件第一册第四章格式IV-9.资格证明文件格式的要求填写和提交有关资料。
*（2）投标产品的原产地在关境外或为由境外制造商在国内全资子公司生产,投标人必须是制造商或制造商直接授权的代理商（进口产品开标前未进入中国关境内的，投标人必须是在关境外注册的公司；进口产品开标前已进入中国关境内的，投标人必须是在关境内注册的公司），如为代理商投标，代理商投标需具备以下条件：
①必须提供制造商对本次投标的唯一正式授权书（授权书必须由制造商直接授权，不允许二级授权）；授权书中应包括“制造商对产品及售后服务质量负责”的相关内容和制造商联系人及联系方式，并确保制造商联系人能够协调处理项目执行过程中的重要事项。（对于此要求，制造商也可在授权书外另行出具书面承诺/说明），否则不接受代理投标；
②投标人在当地依法登记注册的时间距开标日满三年，需提供营业执照等注册登记的有效证件复印件；
③距开标日三年内，投标人须具有同类产品至少两个不同年份的销售业绩，需提供证明其销售业绩的合同或订单复印件（至少提供2份不同年度的合同或订单复印件）或用户证明，并由投标人代表签字或盖章。
*（3）投标产品的原产地在关境内的投标人必须是制造商（投标产品由境外制造商在国内全资子公司生产的，可除外）。
*（4）投标人未被暂停或取消投标资格。（根据《中国石油天然气集团有限公司投标人失信行为管理办法》的规定，失信积分达到8分时，暂停失信行为人投标资格半年；失信积分达到9分时，暂停失信行为人投标资格一年；失信积分达到10分时，暂停失信行为人投标资格两年。暂停期满后恢复投标资格，有效期内的失信记分继续执行。失信积分超过10分或在暂停期间再次发生失信行为的，取消投标资格三年。）上述内容以评标委员会现场在对应网站的查询结果为准。
*（5）投标人未被纳入中石油集团公司供应商、承包商、服务商黑名单。上述内容以评标委员会现场在对应网站的查询结果为准。
*（6）距开标之日前一年内，投标人在中国石油物资有限公司组织的招标活动中对评标结果的无效投诉（包括中石油集团公司机电办处理结果为：不予受理、驳回、维持原评标结果）达到一次及以上的，投标将被否决。
*（7）投标人须提供开户银行在开标之日前三个月内开具的资信证明原件或该原件的复印件（由投标人代表签字或盖章）。
*（8）第八章中要求的其他合格的资格证书。
*（9） 产品制造商须持有国家市场监管总局颁发的《特种设备生产许可证》（压力管道元件A1级），《特种设备型式试验证书》（压力管道元件A1级），且必须具备石油化工行业加氢装置同类阀门制造业绩。《特种设备生产许可证》必须可在全国特种设备公示信息查询平台可以查询，否则无效。
*（10）财务要求:投标人和制造商具有良好的商业信誉和健全的财务会计制度，须提供近三年(2022-2024年)年度经审计的财务报表或会计报表(包括资产负债表、利润表、现金流量表等)。
根据提供的投标人和制造商经会计师事务所审计的财务报告测算，投标人和制造商近三年内未出现资不抵债的情况；未出现被责令停产停业；未出现进入清算程序，或被宣告破产，或其他丧失履约能力的情形；否则投标将被否决。未提供或未按要求提供或数据不清楚的投标将被否决。
(1) All Bidders are requested to fill in according to the requirements of Format IV-9 in Section IV and provide relevant information；
(2) The bidder whose product origin is outside the customs or made by foreign manufacturer's 100% domestic plant must be the manufacturer; Or the agent directly authorized by the manufacturer. The agent shall meet the following conditions for bidding:
① The bidder should provide a letter with official authorized by Manufacturer for the bid；
② the Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
③ The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(3)The bidder whose product origin is within the territory of the Customs must be the manufacturer ( Except production is made by foreign manufacturer's 100% domestic plant)
(4) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches 8 points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches 9 points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches 10 points. If the breach of trust score is 10.5 points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. If the bidder is qualified again at the expiration of the term, the breach of trust will not be cleared and the validity period will be extended for three years.) (6) The bid shall be rejected if the bidder puts forward invalid complaint one or more times in the bidding activities organized by China Petroleum Materials Co., Ltd. within one year before the bid opening date (including complaint rejected, overruled or judged to maintain the original bid evaluation results by the mechanical and electrical office).
(7)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
(8) Other Qualification and Achievement requirements stipulated in Chapter VIII.
(9) The product manufacturer must hold the Special Equipment Production License (Pressure Piping Component Level A1) and the Special Equipment Type Test Certificate (Pressure Piping Component Level A1) issued by the State Administration for Market Regulation, and must have the manufacturing performance of similar valves for hydrogenation devices in the petrochemical industry. The Special Equipment Production License must be available on the National Special Equipment Publicity Information Query Platform, otherwise it will be invalid.
(10) Financial requirements: The bidder and the manufacturer shall have good business reputation and sound financial accounting system, and shall provide audited financial statements or accounting statements (including balance sheet, profit statement, cash flow statement, etc.) for the past three years (2022-2024).
According to the calculation of the financial reports of the bidder and the manufacturer audited by the accounting firm, the bidder and the manufacturer have not been insolvent in the past three years; have not been ordered to suspend production or business; have not entered the liquidation procedure, or have been declared bankrupt, or have lost the ability to perform the contract in other ways; otherwise, the bid will be rejected.
